Re-opening of Service Connection
Monday to Friday from 8:00 A M to 12:00 Noon and 1:00 PM to 4:00 PM

Step 1

  • Applicant / Client: Files applicaiton for reopening of disconnection service connection
  • Activity: Fills up application for reopening
  • Requirements: Application for Reopening
  • Accountable Person: Customer service assistant / Administrative service aide
  • Duration: 5 minutes

Step 2

  • Activity: Check in the database if there is unpaid balances for fees and charges, materials and water bills and computes balances, if any.
  • Requirements: Name and account number of the concessionaire
  • Accountable Person: Customer service assistant / Administrative service aide
  • Duration: 10 minutes

Step 3

  • Applicant / Client: waits to be called by the cashier or office collector for payments.
  • Activity: Submits application for reopening with computation of balances, if any.
  • Requirements: Application for reopening with computation of balances
  • Accountable Person: Customer service assistant / Administrative service aide
  • Duration: 3 minutes

Step 4

  • Applicant / Client: Pays balances to the cashier or office collector
  • Activity: Issues official receipt
  • Requirements: Application for reopening
  • Accountable Person: Office collector  or Cashier
  • Duration: 10 minutes

Step 5

  • Applicant / Client: Returns the paid application for the reopening to the clerk processor
  • Activity: Accepts paid application for reopening and Gives schedule for the reopening of service connection.
  • Requirements: Paid application for reopening
  • Accountable Person: Customer service assistant
  • Duration: 3 minutes
